How does the “fillable PDF” feature work? Do I need special software?

Some of our files use standard interactive PDF technology. To type directly into the chart, simply open the file using Adobe Acrobat Reader (which is 100% free and already installed on most computers). You can also download it here.

Once open, just click into any of the subject boxes, type your prompt, and save the file before printing! If you prefer a handwriting approach, you can skip this entirely, print it out completely blank, and fill it in with a pen or marker.
